The Situation component is the immediate, present-state snapshot of the patient. It answers what is happening right now, who you are calling about, and where the patient is, in a concise way that signals urgency and sets the context for the rest of the message. The aim is to quickly convey the current issue so the person receiving the note knows what needs attention without wading through history or plans.

For example, you might briefly state the patient’s identity, location, and the acute problem, such as a patient in room 210 who is acutely short of breath with an oxygen saturation around 85%. This is distinct from background history (which belongs in the next component), your assessment of the problem, or the actions you want taken. The Background covers clinical history and context, the Assessment is your judgment about what the problem likely is, and the Recommendation outlines what you want done next.
